Individualised Support Plan Based on your assessment, a personalised care plan is developed. This outlines: The services you can access Your care goals and preferences The type and frequency of support Any equipment or additional supports required Your plan is designed to support independence, wellbeing, and quality of life. 3. Funding Allocation You will receive a funding budget aligned with your assessed needs. Funding is: Allocated to specific service categories Used to deliver approved services Managed to ensure it supports your care plan This ensures your funding is used effectively and appropriately. 4. Service Delivery Once your plan is in place, services are arranged and delivered by your care provider. This may include: Personal care and daily support Domestic assistance Social and community support Allied health and clinical services Services are flexible and can be adjusted as your needs change. 5. Contributions (If Applicable) Depending on your financial situation, you may be required to contribute towards certain services. Support at Home services fall under three main categories:
Nursing, physiotherapy, occupational therapy No client contribution is required for these services, as they are fully funded under the Support at Home program.
Depending on your financial situation, you may be required to contribute towards certain services under the Support at Home program. Typical contribution example: Independence Services (e.g. personal care, mobility support, social support): approximately 5% contribution may apply All contribution amounts are determined based on your individual assessment and government guidelines.
Everyday Living Services support your daily comfort and help maintain a safe and well-functioning home environment. These services may include: Cleaning and household tasks Gardening and basic home maintenance Meal preparation or meal delivery ?
